shore tile by the western most SB at A24 (Uterus terrain) has a hill in the water.

No. Island implies land. This is a coastal tile which contains a vertex not at zero alt, i.e. hill in the water.

It's just a VERY isolated and extremely slow moving tidal wave?

Or perhaps it is an unusually high localized tide caused by the ultra intense radiation from the sun?
