anybody ever watch 8mm?  that movie made me not feel right for a couple days.  i would consider that disturbing.

"Kids" was extremely disturbing.

"Faces of death" is pretty bad too, but more of a snuff film/documentary.
Some will fall in love with life and drink it from a fountain that is pouring like an avalanche coming down the mountain

I was tramutized by Jaws..   As a small child I lived in Tennessee and had nightmares of a crane dropping my house in the ocean and sharks swimming in my bedroom..

Pink Flamingos deserves a mention, particularly the fat women in the babies play pen, and the chickens and the things done to chickens....in fact that film haunts me to this day.

I kinda agree with Killnu. 8mm was kinda disturbing.

After Cease To Exist

With a soundtrack by the ever-disturbing Throbbing Gristle, and the very slow, methodical castration scene, this is probably the most disturbing movie I've ever seen.

The worst bit really was that with weirdos like TG involved, you can never be really sure that the castration wasn't real...

Kudos to TG for producing the wickedly misnamed "20 Jazz Funk Greats" LP, though: complete with awful 70s easy-listening Jazz band cover, one can only hope that somewhere a copy still lurks, badly-catalogued in an easy-listening section, waiting for a unsuspecting buyer in search of shopping and lift music...

I've always thought the movie se7en(seven), with Brad Pitt and Morgan Freeman, was quite disturbing.  I will never forget that ending.
