I have only used one and have been happy with it so far: Bugzilla

Ther's another one called "Mantis" out there, but I only got to use that one as a "user" when I was working at the EC, both have web interfaces.

At one time, around here, we had a pretty bad bug tracking problem.
Most of the area is beach-like sand, so it made keeping carpet clean a neverending task.
We finaly found a company that made little tiny shoes. We purchased a gross of said shoes  for the bugs and asked them, politely, to remove them at the door, before entering.
Worked like a charm.
The bugs took to wearing hats, jewelry and other accesories own their own. Fashion statement?

At my job we use DevTrack.  Its very configurable and can adapt itself to many situations, though I think its pretty expensive.
