Have there ever been any fatal accidents resulting in a pilot leaving the "yaw damp" system engaged during take off or landing that you know of?

Beats me.

I've landed with it engaged...I lived.  I think just about everyone at my company has...no big deal especially on nice days.  The winds were calm and I never even noticed until I went to turn off the runway.  If it was windy you'd notice.

As far as taking off the yaw damper isn't turned on manually (it's automatic in more advanced aircraft like the C-750) in the straight wing Citations until after takeoff and I really don't see how you could take off with it engaged.  You test it in conjunction with the autopilot on the taxi and it for all intents and purposes keeps you from turning using nosewheel steering.  You wouldn't be able to make the tighter turns to line up on a runway with it engaged.

In the 550/560/560XL when you hit the autopilot button the Y/D comes on with it.  Electric trim switch knocks off the A/P leaving the Y/D.  The AP/YD disconnect on the yoke knocks them both off.

That's kind of a whack question.  Do not fear the yaw damper.  If you're worried just hit the little red button like you've got OCD.

There are lots of people that could answer your question better than me.  Try the NTSB records, other members of this and other boards.  I've flown airplanes with a Y/D for not quite a year so I'm hardly an expert.
