Ok, so lets say you have run pingplotter and found a reoccuring problem down the line with one of the hops.  

What is a good way of passing this information on to the provider?  For my situation, it's one of the servers that is owned by my provider.

Contact your ISP armed with the information.  The immedaite support will tell you they are only responsible for the immedaite connection to you.

And there is some truth to that.  That is the first line support's only responsibility.  So you ask who do I need to contact to get this resolved?

Rinse, repeat, until you get to the person who may actually be able to do something.
