I recall reading an account where Robert Tuck was supposed to take a Typhoon up for a test flight after some engine tweaks had been done to it. The Typhoon was having a lot of teething issues and engine problems, so various things were being tried to improve reliability.
The aircraft was idling, ready to go when a phone call from group came in for him, so he took the call abd told another pilot, Dack, to take the test flight. A short time later, still on the phone, he hears a tremendous impact outside. Dack's Tiffie went straight in at over 400mph. They found a piece of Dack's liver and on testing it they tell Tuck that it had enough carbon monoxide in it to kill an elephant.
