The thing that a reinstall fixed the problem for awhile leads to think your hard disk is failing in a continuous process. The reinstalling procedure might have bypassed existing faulty sectors during formatting, but new sectors are starting to fail. The Ultimate Boot CD has test programs specific for most hard disk brands, Seagate's and IBM's tests are working with those without a test of their own. If the test fails but gives you the opportunity to repair the disk, bear in mind that such fix is mostly only temporary.

This could be a tough one to nail down.  It could be a hard drive issue, a power supply issue, a RAM problem, or a video card RAM problem.

are you running triple monitors on that video card? --->< Matrox TripleHead2Go Digital Edition>

if you haven't done it already, try removing it or turning it off then see what happens...also there was a windows update put out last week that may be a contributing factor, especially if you're running kaspersky anti-virus...

i'm dinking around trying to figure out if there is more to that microcrap issue...

Sure sounds like a PSU about to take a dirt nap. You will know for sure if audio anomalies start to show up, but why wait? Just have the PSU checked out.
If you like the Sick Puppy Custom Sound Pack the please consider contributing for future updates by sending a months dues to Hitech Creations for account "Chalenge." Every little bit helps.

Yet another thing to check: Since it worked after a reinstall, what would happen if you did a rollback to a date when all still worked? If things start working again, the culprit is one of the updates installed after your initial reinstall. You probably know that you should avoid drivers from Windows Update, at least now it's been said.

The WD Dia tool says everything is fine with HDD. I could check PSU as well, but not yet. I loaded AH on an external HDD run with USB 3 for now and haven't run into the issue yet.

Yes I run 3 monitors and the effects are the same with or without them in the system.
