The USS Stewart of the Asiatic fleet after a dry dock accident . The ship was subsquently captured and became the Japanses PB-102.

 I am often quiet impreased with the Knowledge leval of people on this Forum, many picks I post I think...no way will they get this one....and find that in less than a minute the answer is up:)
