Title: Hi-yo Kidney Awaaaaaaaaaay!
Post by: Toad on October 30, 2004, 09:51:02 PM
My wife and I will start a leisurely drive to MD Anderson hospital in Houston tomorrow.

Will stop in DFW and spend the night , then on to Houston where I'll meet my brother. He'll stick with my wife and I through the operation and part of the recovery.

Doctor's appointment is Tuesday at 0930, surgery as fast after that as I can get them to agree to do it. I could have had it done locally last week, but I held out for Anderson as they have an exceptional reputation as a cancer center.

I appreciate your prayers (feel free to add me to any prayer list you know of) and positive thoughts and general "good vibrations".

My wife will call Rude and he can post here.

I had a few more tests to check individual kidney function and such and they came out pretty good. There's a slight chance they may be able to save some of the kidney but I'm not counting on that. They still have to do an eyeball/hand check of those lesions on my liver too.

There's only one way to tell........ here we go.

Good luck Toad. Good decision in going to Houston. M.D. Anderson is one of the finest in the world. Dr. Denton Cooley performed surgery on my Father there in '92.

Less than three hours to go before Mr. Toad's consultation. I'm hoping Rude or maybe Mrs. Toad might print off this thread to give to him to cheer him up.

Well, Toad - I'm sure you'll be back home soon enough. I'll be looking forward to the next gun thread. ;)

A treat for you when you get out -

Toad's consultation/treatment is definitely the most important thing happening in America today. Are we all agreed?

Hope the surgeon isn't Scottish. Would hate to think of him looking inside and saying "I kidney find anything wrong..." <- said in a Scottish accent. Too corny by half...

Post by: Toad on November 03, 2004, 11:31:26 PM
I went over to MD Anderson yesterday with Jean and brother Bob. It's a VERY impressive place and it really, really seems they have their "stuff" together and very organized in small boxes, if you know what I mean.

They had us checked in and registered in 10 minutes, I had my basic "physical" done 15 minutes later and we then saw the Dr.'s assistant for the recordkeeping/paperwork/background stuff shortly there after. Also got a cheap thrill here. I don't know how many prostate exams I've had over the years in the AF and during FAA physicals but it's been a bunch. This was my first one by a woman! Unfortunately, she was an old nurse and looked nothing like Angelina Jolie. The good news is her fingers were way shorter and thinner than most male FAA docs!

Then they took all my films off for the Doc to view. He came back and I got a short exam and some more questions and then he laid it out.

He says the tumor is too big and too close to the "ductwork" in the kidney for any reasonable chance of saving some of it. He also said there's absolutely no data that shows saving part of the kidney, a partial, makes any difference in the long term chances of the patient. There are risks to a partial, like bleeding and uring leaks though. In his opinion, there was no reason, given the size and location of the tumor to risk the bleeding or leaks. So, he wants to do a "hand assisted parital laparascopic nephrectomy at sometime on Monday.

I told him that I wasn't going to second guess him so let's get it done. I had questions about the lesions on my liver and pressed him on those. From all of my tests and the biopsy, he feels there is no cancer in the liver tumors. He thinks they are all hemangiomas. He said he would just not worry about it and he said looking at and hand feeling the liver during surgery really won't confirm they are not cancer either.
So, no advantage to doing an "open surgery" with the great big cut over the laparascopic/hand assist with a 3" cut.

He said he could order an MRI that would definitively tell me what's on/in the liver if it really worried me. It DOES worry me and I told him so. He said he wouldn't normally order it but if it would make me feel better he would. So, I'm getting the MRI on Thursday, because I want to know for sure.. or as sure as I can be. You don't get to be an "old pilot" without being a methodical, careful, risk minimizer.

So, here's the plan. Wed.. see the sights. Thursday, MRI and consultation with the anesthetist. Friday, Saturday.... see the sights. Sunday... drink the stuff that cleans out your internals..stick close to the bathroom. Monday, surgery. After that... we'll see, won't we?

I have limited internet access at this hotel, so I'll be online somewhat.

I'll keep you updated.

Any suggestions on cool stuff to see/do in Houston?

Hey!

Scoot yer chair up to that little punk-arsed kidney and look it straight in the organ. And say to it "You are not the boss of me."

Then wink at the Doc and tilt yer head in such a way as to say "Yank that miserable shrecker."

Then when she's all wrapped up, say a few words to the Gawdamned almighty.

Not because it would do any good, though it may, but just because you'll feel better after having done it.

Floyd's Cajun kitchen...on Shepard or Durham south of I-10...

If you're in the mood for some good pizza - try Collina's italian kitchen - great woodfired pizza - and pretty decent other dishes.

If your near the med center...Rice Village, off Kirby - theres  bookshops, all sorts of stores, and I think a hobby/toy store for the "elder" kid i believe....maybe it's a train store..food places around also.

See if the game is in town on Sunday :)
Also - I believe CAF has some stuff down in Galveston..45 min drive

Toad the Lone Star Flight muesem is located in Galveston at the airfield, decent collection of warbirds.  Located across the field is Moody Gardens (a big glassed in pyramid).  You might want to check out Johnson Space Center , if you do Kemah is a little port/town adjacent that has some good seafood joints.  As far as downtown or in the vicinity of the Med Center there are a plethora of restaurants/bars etc , just depends on what you're looking for.  Hope this helps.  Best of luck :aok

All I will say is there is some doubt now as to the actual situation he is in. It won't be definate until after Monday morning surgery and a pathologist report is done.

Post by: Toad on November 06, 2004, 09:44:45 AM
Ah, Heck Mav..... ya spoiled the surprise!  I forgive ya.

OK... I'm not big on mystery so here's the latest:

The Doc called with the results of the MRI; the results are "mixed and unclear".

I have 8 lesions on my liver. The Radiologist classed 3 of them as cysts which are no problem. He classed 4 of them as hemangiomas, the knot of blood vessels which are no problem.

The remaining one is a 1.8cm one that "enhances differently than a hemangioma". I should have asked him what it DID look like, but I admit I was a bit speechless.

The kidney doctor feels I have papillary renal cell carcinoma in my right kidney. IF this hasn't spread, that's a good thing. Papillary that hasn't spread has a bit better outcome IF you get it all when you take the kidney.

Doc Wood feels that it is very unlikely that a single isolated tumor metastasized from this. He says that usually, if it metastasizes, it does so in multiple areas at once. He characterized the chance of this being cancer as "remote" in his opinion and experience. He thinks it is probably benign as as single liver metastase is "very unlikely" and he has "never seen it before".

He then talked about what we do now. If it isn't cancer (in the liver), the laparascopic kidney removal as planned is the best answer.

If it IS cancer, there's a fork in the road.

There is proof that you live longer, even if it has spread, if you remove the primary tumor. So this thing has to come out anyway.

Now, we can get a liver surgeon in and do an "open" surgery instead of laparascopy. The liver surgeon would ultrasound the liver and biopsy the "concerning" mass and then Wood would still remove the kidney.

Or, we can do the laparascopy on the kidney and then in 3-4 weeks repeat the MRI and see if the "concerning" mass has changed. If so, they can biopsy it much the same as they did the hemangioma last time. If it hasn't changed, they just keep watching it. This is what Wood prefers to do.

Here's why: he doesn't think this is a cancer. He thinks it's benign. I guess he wants to see exactly what type cancer is on my kidney before getting too aggressive with the liver.

The other factor here is that IF it is papillary renal cell that has spread to my liver... there's really nothing they can do. It does not respond to either radiation, chemotherapy or immunotherapy. You can sometimes cut it out of the liver, but it will be back.

Bottom line is it it isn't cancer, there's no need. If it is, they can't really help me anyway.

Cool deal, huh? Needless to say, while I was feeling pretty good about this whole deal, this news is definitely a downer. I thought I had found the get out of jail free card and only paid a kidney for it. Now, that's not exactly the deal again.

Anyway, I told him that, as I said, I wasn't going to second guess him. I came here because I figured these guys knew what they were doing.

So we go as planned on Monday, 0730 Central.

I appreciate your prayers and positive thoughts.

Adios, muchachos!

Off to the surgery tomorrow. 0545 check in, 0730 slicing and dicing.

Be a while until I'm back on the boards I think.

Rude says he'll post the word.

Thanks again for your support!

Just an update.....

John went under the knife this AM and his wife just called me....he got thru fine with minimal bleeding and will be dining on morphine for most of today.

The tumor itself is otw to pathology which will take a couple of weeks to get the results back.

Just as a favor to me as John is my best friend here in KC, take a sec out of your day and ask God to be with him and to restore his health....both he and I would be thankful:)

Will keep everyone up to date as news presents itself.

I spoke with John on the phone this AM....he's doing the recovery thingie and is kinda hurtin, still his spirits are good.

Here's the plan....the hospital will release him this Friday and he will return home to KC. They are awaiting the pathology report on the removed kidney so as to determine what type of cancer had invaded the kidney....they expect the results back at the earliest Friday, the latest would be next Monday.

He will return to Houston for another series of scans and a possible biopsy of a growth on his kidney....this is expected to be benign, however, they just want to make sure.

Again, John is very grateful and touched by your prayers and support....please keep them coming and let's all stand together in faith for John's health to be restored.

One single truth? The best thing about AH is the people that fly it....again, I thank you guys as well.

When he gets home, mount that LCD monitor above the bed, get a monitor cable extension, put his headphones on, ductape his joystick to one hand and his keyboard to the other...  Load up AH and enjoy the recovery  =)

Just be sure to RTB before the drugs kick in...

Seriously though... great news that he's doing better.  All these guys are pulling for him, and he sounds like a remarkable individual... I'm sure he'll come through with flying colors!  \

If he's bored & in any kind of mood to play, let us Houstonians know! I'll run him a laptop & joystick.

He is in excellent hands at least. Houston hospitals are great. I've had the (un?)fortunate luck of being their guest a few times.

Man - I just read this thread.  I can relate very well.  I went thru the same thing a few years back - lost my right kidney.  Docs said it had a huge mass in it, no function - could be cancer - had to come out, etc.  Had it out, coded the day after surgery, woke up in ICU a few days later.  Turned out not to be cancer - just a huge blood clot where a kidney stone had ripped a vein or soemthing.
Our prayers are with you for a complete and speedy recovery!  Take advantage of the morphine while you can!:)

I think it's a long drive back from Houston to Kansas City (more than 600 miles) so if Mr. and Mrs. Toad started home on Friday, they might have wanted to take two days over it - probably due back sometime today.

Any news about the tests? Is it confirmed benign?

And how is Toad doing?
Title: Hi-yo Kidney Awaaaaaaaaaay!
Post by: Toad on November 17, 2004, 10:36:22 AM
First a big "thank you" for the prayers and positive thoughts. I think it works and I know I felt it. I felt very protected and safe as they rolled me into the OR. Now, there's only one reason I can see for that feeling when you're headed in to cancer surgery.

So, thank you all and keep up the good work. There's still a few things to be resolved.

There's no pathology report as yet. That takes 2 weeks they say. I'll be back there in 3-4 weeks for a standard clinical follow-up, standard "how's that other kidney doing" blood tests and a "standard" chest CT scan.

I will also get a non-standard MRI to check the "concerning" liver lesion. If it has not changed in size, shape or radioactive "enhancement", they will keep watching it with regular MRI's I guess.

If it has changed, they will most likely biopsy it and decide on further action from that. Teh Doc thinks it is another hemangioma, and that's what I'm praying for; seems like the lesser of evils, for sure.

I am in a bit of pain, but the pills handle it OK. I have a 4 inch North/South scar on the right side of my navel and about four 3/4 inch scars where they stuck in the "lights, camera, action!" stuff.

I mostly sleep and try to walk as much as possible. I only eat a little bit; nothing is really appealing. I have some breathing exercises to do since I lay down so much.

Things get a bit better every day.

Rude says he's going to come over and try to reprogram my analog stick. I can't really bend and twist under the puter desk to do much swapping to the old W98 puter.

Maybe I'll get up in AH before too long.

Thanks again for all the support; it may seem like a little thing to you but I assure you, it's been major for me.

Thank you all for your kind support.

I had a major operation 4 years ago and can tell you the worse days were the 2 or 3 following the operation. Each day after that got better. Your craving for food will come back and even that is great. I still remember my wife taking me to lunch near the hospital when I went in for a followup check and how nice the meal was. Every day was a gift (and still is).

Get better and stay positive.

Welllllll... I don't want to bore people and I sure don't want anyone thinking I'm home crying in my beer.

I had a recheck at Anderson on the 6&7th. Scanned my chest, abdomen and pelvis and said no new lesions and all those things in my liver look the same.

Doc said go forth and get back to doing what you do.

There still is that little problem area in my liver. Remember that tumor that I mentioned as "The remaining one is a 1.8cm one that "enhances differently than a hemangioma"?

Well, what the actual MRI radiologist report said was that it  looked "typical" for a metastasis from the kidney.

My urologist/kidney suregeon however, says there is almost "no chance" of that given the type of kidney cancer and the stage/grade. So he's saying the chance is as close to zero as there is.

They want to rescan me in 3 months and see if that lesion changes. If it does, they'll biopsy.

I'm having a detailed chat with my local doctor who is a very sharp guy this afternoon. He talked with my surgeon and initially wanted immediate biopsy but after the chat, he now agrees with the surgeon.

I'm not really sure what it all means but if it IS kidney cancer that metastasized to my liver, there isn't much to talk about.

I can't control that, so I'm just trying to get on with my life.

MD Anderson is supposed to be real good at cancer and their guy says studmuffingeddaboudit.

I'm trying to do that.

Still cheerfullyl accepting all prayers, good wishes and postitive energy!
