You're allowed to smoke on a commercial flight as long as you're at least 50 feet from the door, and 6 miles down. :)
And FWIW, one could never get the door of a commercial flight open...cabin pressure.

Last time I flew from Japan to Yurop, they busted 4 guys smokin' in the toilets (at the same time).
They "must" know there's a smoke detector in there... don't they?

Fortunately Russia is a smoking country. It's always possible to talk to a stewardess and smoke somewhere in the back. In Tu-154 they gave me ashtrays. In An-24 (small turboprop) we smoked in the tail compartment, between control-rods.
As for smoke detectors - AFAIK it's possible to cover them with something like a condom or a polyethylene wrap from your lunch ;)

They should have acomendated her. Fly down to a level where people can breathe, open the door, and let her out.

Interesting you point this out b.c. I remember when I was on the 154 there was a guy doing the exact same thing. But was mentioned there was some sort overboard waste gate b/c he was in the back? I dunno, different experience all together. Sleeping on them though, was a matter of propping yr head against the window and listening to "wooooowwww woooooowwwwww woooooowwwwww." Great for head massage though.
