definitely looks like a driver issue

That driver is pre-Catalyst somewhere around 2/2002.  Get the Catalyst 4.12 drivers and be sure to remove this old driver before installing the new one.

Also be sure to install the control panel with the driver.

Your driver version should be 6.14.10.6497 after the update.
