I'm sure at least someone has had the unwanted experience. I'm looking for advice on what to consider, who to see, etc... Are there any leading pioneers in the field? Any experience with the various medicines?

Suggest a neurologist visit. Weather or not medications are needed depends on the condition and where the condition is in its natural progression. Of course all medications have side effects. The question is weather the benefits out weigh the risks.

My step mother was diagnosed with it about 15 years ago.
She is now used as an "example" of how to successfully keep the symptoms at bay, by a doctor in Baltimore.
I'll check with her regarding medications and recommendations for doctors, but there are two things she does religiously that have helped her:
1. She eats "right". No junk food, lots of greens and takes a number of multi-vitamins.
2. EXCERCISE! Critically important. She walks about 2-3 miles a day, and works out on a treadmill and with weights (small ones obviously). She is fanatical about it. You can really tell if she misses her excercise on any given day...she shakes badly and has a hard time getting up etc.
It is a nasty bit of work Parkinsons...but it can be managed.

Rabbid.
I was given two names of neurologists at Mass General in Boston:
1. Dr. Caveness
2. Dr. Flaghty - not sure about the spelling on this one.
Currently my step mother is taking Sinemet 25/100, but this medication is very specific to the individual and whomever has been dignosed in your family may not need such a strong medicine.
Excercise, excercise excercise. My step-mother wanted me to reiterrate the point.
