Title: Any ideas on a mail list server?

OK,

Say I want to push a newsletter once a month and I want users to be able to opt in and out.  What are my options?  A  service?  A mail server?

You could use Yahoo groups. It's free. http://groups.yahoo.com/

this is for a business so I think I'll need something else..  Good idea though.

In that case... PHPList (http://tincan.co.uk/phplist) on your own server.

I use SurpassHosting.com for my webhost. It has PHPList available. I suspect that many other hosts will do the same.
