I can't get this to work at all. I've tried mashing T, F12, and resetting all the keys, and I can't get any sort of response at all from anyone! My mike works in Ventrilo, so I know the thing works in theory.

Couple of things to check.
In Windows make sure that in the recording section of sound properties your mic is selected.
In the game tune the audio channel to yourself with a dot command like this
.radio 5
this will give you feed back on how your audio sounds. You can alt-tab out to windows to adjust the mic volume to taste. You may need to check the +20db box in the advanced properties of the mic in windows.

I've done this and I read myself loud and clear.
What channel do you tune your voice channel to?
I have that the moment:
1 2 3 4 and V channels....
1 is country, 2 is room, 3 is squad, 4 is all and V is mission
My problem is that I cannot seem to tune 'V' to 'range' or room or whatever it is so that i'm able to speak to all friendlies in radio range.
My only options for channel V are Squad and Mission.
Any suggestions?

Xasthur, you can *always* talk to people on range, you dont need to tune that channel. Just hold down F12 (instead of T) while talking.
The V channel (tunable vox) is for other channels. Squad (red) voice, Mission. Most Squads also have a number channel (in the 100-199 range, thats on team only).
The channels 200-299 are also voice able and go cross teams (ie players from different teams can tune it and hear/talk/type on it). The most common one is 200 for text (in replacement of the disabled All channel).
For those number channels: only players that have tuned it can read it. And they can only HEAR it if the have it tuned in V chan.
You can also tune another player to voice private (by writing his name into the V chan box).
You can also use the V chan as another text channel (ie type on it). Just use the SHIFT \ shortcut to open radio.
Channels that can be read even though they are NOT tuned: (All), Country, Range, Squad, the CM channels (gold and blue) and HTC official channel (turquoise).

Edit: With the new setup, the red Squad text channel is cross arena. You can also page another player with .p gameID . This will send a private text (white) to the player in question.

I found the nature of my problem, my Function Lock key was on (my keyboard has no warning light for the F-Lock key) and this effectively turned off my F12 key, so my Tuneable V channel worked, but not the F12 key range channel.
Now I just need to improve the quality of the signal from my cheap-o microphone
