A while back, an F-16 pilot inadvertently strafed a school in New York with his 20mm. On the F-16s control stick ,the trigger is used for more than one purpose. Depending on which menu you have pulled up, the function of the trigger changes. For example, in the New York situation, the pilot was trying to use his GPS waypoints, but had the wrong menu pulled up. So instead of getting his new waypoints, it fired the cannon. Maybe something like this caused the training bomb to be dropped.

Well offhand I'd say that there is a pilot, crewchief and at least one ordinance hanger that are going to be rather uncomfortable for the next several days. At least until the source of the problem is identified.

Reading the comments under that story makes me think there are a bunch of
folks in need of a clue in Tulsa. From time to time things fall off aircraft, unfortunately
this time it did damage. Fortunately no one was injured.

Things like this has happened to me too a few times. Usually when i have to reinstall AH and the buttons are not mapped correctly yet.
Im guessing the pilot had a reinstall issue.

I remember reading that the janitor of the school actually heard the rounds hitting the school building.

a. No wonder pilot training takes so long.
b. Yes, ladies and gentlemen, for making a trigger do more than just launch deadly projectiles, put your hands together for the new leader on the Dumbest Design Hall of Fame!
c. (Deep in underground missile control room) Now let's see, where again is that button that switches the trigger function from Start the Coffee to Launch the Nuke? Or is it the toggle under the seat? Do I use second finger right hand or third finger left hand? Function L5 or R8?

The crew chief has nothing to do with the ordinance and it would be incredibly hard to screw up the loading of a BDU-33 to the point that it accidentally dropped off the airplane in flight, in fact I'm not sure it's possible.
It was probably either pilot error or a malfunction with the aircraft itself.
