What is the application procedure for the Achievement Star once you have the 25 item qualification?

It's no longer 25 items. Achievements have different reputation values, and you need to reach certain reputation levels to get one (or more) stars. Check your achievements list and hit the "reputation" button. You will see that your current reputation of 450  is slightly short of the required level for the first star. You need 1-5 more achievements (depending on their rep. value).
