Are you using "auto-takeoff"? It tells you what to to.
If you are trying to take off manually, then use <SHIFT><+> (the plus sign on the keyboard, not the keypad) to go to full throttle.
Do not use the RPM commands, thinking they are the throttles.
You can see and test what is mapped to your keyboard in the Options->Controls->Map Keys panel. If you are using a non-standard keyboard, you may have to use the "Fix Keys" button (it tells you what to do) in the Options->Controls->Map Keys panel to fix the key so the game will recognize it. That is only if you press it and the virtual keyboard does not show it being pressed.

Just a note - the instructions in the auto take-off do not necessarily work with international keyboards. IIRC I had to press a totally different key combo on my scandinavian keyboard and that took some time in trial/error.

That is when you use the "Fix Key" function in Options->Controls->Map Keys. When you go into that panel, the virtual keyboard will who what the game thinks is being pressed. If it does not match, then you can use the "Fix Key" button to teach the game the keys.
It has little to do with the nationality of the keyboard and more to do with how the manufacturer has altered the key scan code from the standard 102 key layout.
Most laptop keyboards do not comply with the 102 key standard, as an example.
