So I receive this letter from my step-father. He informs me that I am a beneficiary of an IRA account. This happened in 2012 and about 4-1/2 years after my mother passed away.
Since my step-father only provided the name of the IRA company and very little else, I called up the IRA company for more information. After talking with an account representative, I learned that I had sent them a Disclaimer of Interest about 3 years earlier. The weird thing is I have only known of this IRA account for less than a week, imagine that?
The document below is a copy of my Disclaimer of Interest with my forged signature (blotted out for privacy issues). I was told this Disclaimer of Interest was rejected because of a 9-month rule. This type of document needed to be sent within 9-months of a decedent’s date-of-death, otherwise it is automatically rejected. Had the IRA company NOT reject this document the funds would have gone directly to my step-father instead of me.

Absolutely, go to the Police and file a report. Additionally, you may what to notify the FBI dependent upon what the local PD says. If it was sent across state lines, it becomes a federal crime.

I did report this to multiple police stations and one sheriffs office. I also contacted and sent a copy to the FBI, local DA's office, my states taxation agency, and the IRS.
I also retained an attorney and applied to have myself appointed as the executor of my mother's estate to have full access to all information. My step-father produced a Last Will and had the court appoint him as the executor, this was around May 2012. After my step-father was executor for 3 months my attorney requested a Discovery to produce all the estate information. My step-father and his attorney stonewalled the Discovery request and after another 3 months my step-father submitted an affidavit to the probate judge asking to close the case because of no probatable assets, the judge approved closing of the case. What is strange about the probate case is my attorney nor I was given any Notice of the affidavit to close the probate case.

Mate, if you have a lawyer, let him know you posted this on a public forum. Even with all that blanked out info, he would most likely tell you to pull it down. Its possible to use postings like the above against you.
