Thanks to Lyric1 for finding me the info on this unusual Spit scheme.
This Spitfire Mk V was flown by 4 Squadron of the South African Air Force from Sicily in August 1943. With its PRU blue over Mediterranean blue scheme it was likely operated in an armed reconnaissance role. The scheme features the SAAF's version of RAF roundels with an orange centre and a springbok for noseart.
