this is the second time iv refueled in FSO, turned onto the runway and exited my plane only to get a ditch?
what is the proper procedure?

Not having any footage to review, make sure you are on concrete before you stop and exit your plane.
If you have film of it, I'll gladly review to see if I se anything else.

I always film, and I always open clipboard to verify I've got a green end sortie.

Ya. When you open up your clipboard, the "end sortie" text changes color depending on if you're safe to land without a ditch. It will be red in flight, or when you aren't on the runway correctly once landed. Once you're on the runway, fully stopped, the red, turns green.
It works in GV as well. If there are no bad guys within 6K (i think), it will be green when you're stopped. If it's red, the only to get a good landing (green end sortie) is to be inside a hanger (typically).
