Doesnt always happen but does seem to happen a lot.....
I'll log in and fly for a while, then maybe I'll notice a squaddie on and try a radio check but get told I'm REAL quiet. The only way to fix it is ALT-TAB out, open the volume control and move the sliders for both mic play control and mic record control. The sliders are both up to max but I have to move em both to minimum and then back to max. I have to do this to the mic level on the play control screen and the record control screen, even though on the play control screen mic is ticked as mute. On both control screens mic boost +20Db is ticked under advanced but I dont have to untick and retick to fix vox. Then ALT-TAB back in and the vox will be working fine.
Why do I have to do this and how can I stop it?

and play control.....

Click the advanced tab under microphone, there should be a 3d properties box to be checked at the bottom(dont have it on this puter at work so can't take pic and add) , check the box and see if that cures it, had a similar prob and that fixed it-hope it helps

On board sound card? try finding newer drivers
-
I had the same recurrent problem with a SBLive 5.1 Xgamer card... I used to fix it by cycling the "micboost" button. Never tried your solution, It might have worked for me too... I got tired of fighting it, so when I bought my new MoBo I got one with onboard sound. Tried it, and have lost sound quality - but I don't lose my AHV anymore. A squaddie with the same card, had the exact same problem too.
SBLive card?

I had same problem as many of my squaddies did with the SB Live, downloaded newest drivers from the Creative site and problem solved, hope this solves yours too.

try turning down your sound acceleration in your direct x setup. go to sound and move the slider from the farthest right position to one over to left. Then check out gameplay and sounds if it is still the same continue moving slider until it works the best! Good luck:cool:

If you have a sblive, then try the newest drivers. They fixed this problem for me.
