Were you using VOX at the time, or used VOX during a session then this happened when you quit AH?

I can't even get into the game.  But probably I had been using vox in a prior session.

You have some corruption somewhere.  Try reinstalling DirectX, then the video card driver and see if that corrects it.  You may also need to reinstall the sound card driver as well.
