Off Formosa, October 1944, part of the battle of the Philippine Sea. They were part of task force T comprising naval and Army torpedo capable units designed to atack American carier groops, and night.

It's used in Ishigaki-jima (Ishigaki Island) against US fleet in the night of 12th October, 1944.  It was night time torpedo attack. But mission was failure.  20 Ki-67s rolled for a sortie, 3 Ki-67s ditched in China territory, and 8 Ki-67s are MIA.
