does anyone know how to install phpFormGenerator?

Its giving me one hell of a time, i keep getting an internal server error after trying to submit the form, or when trying to get to the admin page

ive CHMOD'd everything that im supposed to, as well.. so frustrating..

i think it might be something along the lines of even though i can set 777, im actually not allowed to with my rights and i have to have my hosting service do it from their end.. maybe..
