This should be an easy problem for someone to fix (except me)  

Athlon 800
Asus A7V MB
GeForce2 MX 32mg vid card

This contraption seems to run fine in all the resolutions except for 640x480.  I know, I know, why would I want to run anything in 640x480??.......Still have kids that like to play some old stuff that only like the 640x480  

Pretty sure I have all the latest drivers....

Any idea why, when I try to set desktop resolution to 640x480, it won't do it??  It refuses to register and kicks me back to my previous resolution setting......

Any help or suggestions would be greatly appreciated!

hmmm...  
Monitor installed correctly into windows?  if u aren't sure, go the manufacturers website and get the latest .inf files.

Refresh rate.. try setting it manually for 640 resolution.
Are u using any tweaks for the MX? (overclocking utilities etc)
