Drane - yes I tried all that. It still comes up windowed at first even when I set max resolution. Always did.

I tried dropping resolution to 1280x1024 ... no joy.

Drivers for the GeForce are the very latest. System has been rebooted and all the usual tricks attempted.

What Dobs describes is probably related because it onset at P2.

This is almost precisely what I was describing, or attempting to describe. I seem to remember that one of the monitors was changing refresh rates to an unsupported mode? Windows was attempting to over rule the Nvidia Control Panel I believe. I can't remember exactly how I fixed it though.

These are all probably related. Whatever was done in Patch 2 affected initialization of the framebuffer somehow on some machines.

FWIW, in the latest patch I can now zoom the launch window (not alt-return) and the game now comes up.
