Do they come out grainy? Or is it somewhat like a picture?

Depends on the quality of the picture your working with.
whatever quality ou get out of your printer is the quality you get in the transfer.

I always print a copy on plain paper first to see what it looks like  then when I am satisfied I'll do the Iron on.

Oh BTW of you have text on whatever it is your trying to print remember to flip it so its backwards or when you do the Iron on it will come out backwards
