I can only speak of the re-enactments I've seen (mostly civil war), and while they were likely 'aiming off to the side', they were still aiming close enough to each other for my father to give you a butt kicking for pointing a gun at someone.

about 45 degrees off is about as close as would have been tolerated.  granted I've only seen 4 or 5 re-enactments but looked to me that if they were aiming to the side it was 5 or 10 degrees at the most.

We also Elevate..

Over long distances, to keep it real, we keep the barrel level..

When we get closer the barrels elevate..
