Should v sync be on or off and what does it do?

It synchronizes the line building to the monitor refresh rate.
21 is only half the truth.

If you have vsync OFF you will get higher FPS listed, but you will have problems with bullets displaying as hits but not hitting (it needs to wait for the full frame to display before it calculates if you've "hit" your target, if vsync is off it doesn't wait and the calculations show hits that never land).
