Ping Plotter help?

I've run ping plotter due to the number of disco's I've been getting, read a few threads on this in the forum and don't think what I'm seeing is good.
I started AH in October 2007 and have had regular discos, howvever this month I'm up to 13 so far. While playing Warbirds for the previous 3 years I don't think I had 5 discos total.
My ISP has been the same Cablevision. So here's a shot of my pingplotter results, maybe one of you could interpret it for me or even sugest what to do with this info. Thanks in advance.

Hope 3 is bad along with 4 and 5. That close to start I'd assume your ISP should be informed.
Far too many, if not most, people on this Board post just to say something opposed to posting when they have something to say.

Most of the problems are in cogento's network.  The trace back to you is also problematic in cogento's network.  Need to get your ISP involved, but this one is going to be a tough one.

International trunk lines being what they are.
