Stuttering/Choppy after Patch 7/8

I've been having stuttering problems since Patch 7, continuing in Patch 8. Never had problems before. I'll be looking around or turning and it will suddenly get a bit choppy.

1920x1200 resolution
Hi-Res texture pack
Object Detail maxed out
Ground vis at ~2miles. I had it on 4miles and tried dropping it down to see if that was it but no change
Detailed ground and water
Bumpmapping on
Shadows set to detailed, using 1024 resolution

What ever happened to preloading the cache?

I've always attributed my stuttering, long before the current version, to loading times.  Preloading used to help.
