You're getting 3% packet loss TO YOUR ROUTER (192.168.0.1 is your router) so it is quite possible that your router is going bad. I don't think this is an ISP issue. Try unplugging it for several hours to give it time to discharge (overnight would work) then plug it back in.. sometimes rebooting like that can fix this sort of thing.
It's also possible that you have a network card or cable issue, although Windows will often tell you 'network disconnected' or the like if this is the case.... or if you are using wireless (Skuzzy will say 'all bets are off') something may be interfering with your signal.. possibly a cell phone, cordless phone, neighbor's ham radio, can opener, high tide, German UFOs, static discharge from your underwear due to new laundry detergent (just kidding, but you get the point).
If you ARE using wireless, try running a network cable from your computer to your router and disabling the wireless and see if it improves. If this isn't an option, you can try moving your router to different locations (a few feet can sometimes make all the difference)... maybe you'll get lucky and find a place that works that isn't too horribly inconvenient.
