Two questions...does it happen in offline mode, especially flying low...and are you using an Nvidia GeForce graphics card?

Seems to happen in offline as well, plus being this computer is 10+ years old doesn't help much.

Also, I think I narrowed it down, what is going on is windows is 'resetting' the driver because of a 'Video hardware error'. To see if this is the reason for you, go to

control pannel->Problem Reports and Solutions->view problem history

From there you should be able to select 'view a temporary copy of these files' where a folder will be opened and there should be a crash dump file.
