Murphys law gets you every time.  One more correction to procedure in the never ending balance of automation and operator error.  I really liked the animation of the socket hitting the ground.  I bet it's a good bit like how it happened.  Funny that a physical switch is what kept the Carolina bomb from blowing.  As they said, good engineering was the safe.  Like a good scientist told me, you got three genes that would need to mutate.  The odds are astronomical.

What I didn't understand is what they could have done about it even if they had been more decisive. They mentioned an emergency unloading of the oxidizer but how could you do that when the whole place was full of rocket fuel mist? I mean, any tiny leak of that stuff from hooking up a hose or what not would be curtains. I didn't understand why everybody fled after the explosion, were they afraid of contamination or were they just freaked out? I also didn't understand the whole thing of going in through the blast doors when they had already gone down the escape hatch. All in all it seemed like no one ever thought there could be or at least never made a plan for how to deal with a significant accidental fuel leak. Those guys in the white suits sure had balls :salute

Just watched it on Demand. I never knew that happen. Despicable how the people who went in were treated after the incident. Thanks for posting Floobster.

If I remember right they violated several safety protocols or it wouldn't have happened. When USAF says fold your underwear exactly six inches believe me they mean it. Oakranger the Titan-II is long gone.
