Check the FAQ,

You will need a  webpage to host the image but here is the code:
JUst remove the _ after the

Note that the http:// part of the image URL is required for the [img] code.

Nope,

can be any image format (gif, jpg, bmp etc...)

Someone also had posted previously, that most free websites have restricted off site linking of pictures....but they often allow TXT files...and if you rename a GIF to TXT it still shows as a picture!
