I've had several friends die riding bikes, and several die in cars. My first was a friend when we were 14 and he failed to stop for the flag-man. He was instantly crushed by an earth-mover. As with anything riding is a matter or responsibility and experience.
I never let my wife ride without PPE, she will not let me ride with PPE either. Just yesterday I watched as a lady pulled out right in front of my wife, after looking us both in the eyes as she sat at the intersection (I was in trail). Defensive driving is an absolute must. It only takes a second of daydreaming (easy to do when riding, it sorta fun), to lose sight of the road in front of you. We always follow the 12 sec, 8 sec, 4 sec rule, it pays.
