Im typing up a agreement now, heres what it would look like:
The agreement that follows was reviewed by both parties, (my name) and (Jeff), and that by signing this document both parties agree to the terms and conditions that this document contains.
On December 22, 2012 at 12:01AM, if the party of (me) is living and able, then the party of (jeff) will give the party of (me) full credit to the savings and all monetary instruments that the party of (Jeff) has ownership to as of the signing of this document, and that are listed below:

On December 22, 2012 at 12:01AM, if the party of (me) is living and able, then the party of (jeff) will give the party of (me) full ownership to vehicle that the party of (jeff) has in ownership under his name.
On December 22, 2012 at 12:01AM, if the party of (me) is living and able, then the party of (jeff) will give the party of (me) full ownership to the household/property that the party of (jeff) has ownership and title to that of said houshold/property.
If on December 22, 2012 the party of (me) is not living and able, then this document is thereby null and void.
This document and all conditions listed above has been reviewed by both parties (My signature) and (Jeffs signature) and by signing this document with the set conditions above both parties agree to these set conditions.
