Something like the F-16 throttle. I'd get a Cougar but the competition on ebay is too fierce.
The Warthog throttle is just too big with the buttons too spread out (and it's too expensive). Right now I'm using the CH throttle. It is extremely comfortable, and the button layout is perfect, but it feels like it only has 2/3 the range of the x52 throttle, and it doesn't have rotaries that I can use for RPM. When flying planes like A2A's P-51 in FSX, that becomes a problem.
So probably the ideal thing would be a CH throttle with a couple of wheel axis added on either side of the three buttons and about a third more throw for the main axis.
Now that I think about it, I don't think I would be capable of building that even if I had the parts, so yeah, asking too much I guess. Oh well, I think big.
